Dental Insurance Information
We accept most Delta Dental, Anthem and United Concordia insurance plans. We will be happy to help you understand the coverage that you have. We will do our best to see that you have recieved your maximum insurance benefits for all covered services, however, please note that dental insurance is intended to cover some, but not all of the cost of your dental service care.
Most dental insurance plans include co insurance, a deductible and other non covered expenses. The fee is ultimatley your responsibility. Please bring in your insurance plan information and we will make every effort to assure you recieve your maximum benefits. Should you have any question about your specific insurance plan, please contact our office.
Think of dental insurance as a gift card with a certain amount of money on the card. When the gift card or insurance is used up the benefits are all done for that year.
Medical Insurance Information
Insurance information about Oral Appliance Therapy
Most medical insurance companies cover oral appliance therapy for treatment of obstructive sleep apnea. There are some policies that have specific coverage maximums or exclusions. It is best to read your policy carefully and speak to your medical insurance representative prior to your visit. * It is imperative that you know your policy, it is your best tool to stay one step ahead of the insurance companies.
As a courtesy to our patients, we will prepare and submit your insurance claim for most medical insurance companies from the office. We will also provide an estimate that will show expected insurance reimbursement , the patient potion will be done at the time of service.
Most medical insurance companies do not have in-net work providers for this treatment. We participate with most Anthem Federal and Anthem P.P.O. policies.
It is our office policy to require payment at the time of service unless prior exceptions are made.
