Your responsibilities as an informed patient:
We will be happy to prepare and submit
claim forms as a courtesy to our patients.
Although our fees are usual and customary
for the community, please understand that
some insurance policies have excellent
coverage while others have poor coverage
for the same procedure.
Payment is expected when services are
rendered. Payment of co-insurance or deductible
is due at the time of treatment.

Office hours are Monday through Thursday,
8:00am to 5:00pm. By special arrangement,
selected Friday appointments are available.
Emergency care is available to our established
